Chow Mein Express was shut down by the Southern Nevada Health District on Feb. 7 after inspectors found 43 demerits during the routine inspection.
Among the violations were staff not washing their hands properly, food unprotected from contamination during storage and by chemicals and more.
SNHD closed the restaurant two other times on April 19, 2012, and March 3, 2008. The restaurant also has repeatedly earned a C grade during inspections that later turn into an A on re-inspection.
The Southern Nevada Health District routinely shuts down restaurants that receive more than 40 demerits. Restaurants have the option of requesting a re-inspection before the next scheduled inspection.
