Mr. Sandwich III at 4626 S. Maryland Pkwy. came within one point of being shut down by the Southern Nevada Health District after a routine inspection on Jan. 15.
Health inspectors found 40 demerits during their visit. Violations included staff not properly washing their hands, potentially hazardous foods stored at the improper temperature, food unprotected from potential contamination and more.
A routine inspection on Oct. 16 last year revealed 34 demerits, followed by an upgrade to eight demerits on Nov. 5.
The Southern Nevada Health District routinely shuts down restaurants that receive more than 40 demerits. Restaurants have the option of requesting a re-inspection before the next scheduled inspection.
