Kung Pao Wok at the Las Vegas Premium Outlets North came within two points of being shut down by the Southern Nevada Health District following a routine inspection on May 19.
Inspectors found food held at the wrong temperatures, food unprotected from potential contamination, single use items reused, ineffective pest control measures and more among the 39 demerits issued.
The restaurant received a C grade with 30 demerits last year in February that was upgraded to an A grade 12 days later.
The Southern Nevada Health District routinely shuts down restaurants that receive more than 40 demerits. Restaurants have the option of requesting a re-inspection before the next scheduled inspection.
