Lee's Buffet at 1510 E. Flamingo Road was shut down by the Southern Nevada Health District after a routine inspection on July 14.
The 42-demerit inspection found that staff wasn't properly washing their hands, food was unprotected from potential contamination, ineffective pest control measures, inaccurate thermometers and more.
Lee's Buffet has been shut down three other times by SNHD since 2012. Last November, SNHD shuttered the restaurant for 35 days after a 19 demerit re-inspection.
The Southern Nevada Health District routinely shuts down restaurants that receive more than 40 demerits. Restaurants have the option of requesting a re-inspection before the next scheduled inspection.
