Ichiban, the sushi restaurant in the food court at Fashion Show mall, was shut down by the Southern Nevada Health District after a re-inspection on July 29 found 20 demerits.
SNHD inspectors found food unprotected from potential contamination, ineffective pest control measures and more. The fast food restaurant was originally inspected on July 9 and received 33 demerits.
While the restaurant has never been shut down by SNHD in the past, it did receive 40 demerits, one away from shutdown, in 2012.
The Southern Nevada Health District routinely shuts down restaurants that receive more than 40 demerits. Restaurants have the option of requesting a re-inspection before the next scheduled inspection.
